I, EUNICE LAMBAINO o f ID NO. 13070478 P.O. Box 7710-30100 ELDORET in the Republic of
Kenya do hereby make oath solemnly and state as follows:
1. THAT I am a Female adult individual of sound mind and the deponent herein thus compete not
and authorized to swear this affidavit.
2. THAT I am the Mother to TONY KIPROP.
3. THAT the said TONY KIPROP has been offered to study at the Learn Key Institute Malta in
Malta to pursue a Diploma in health and social care management
4. THAT I am a businessperson within the Uasin Gishu County.
5. THAT I will personally be responsible for Transportation, Tuition fees, Medical
accommodation and any other expenses he may need while being there.
6. THAT I have annexed herewith my bank statements from KCB BANK Account No’
1335912657/1289906572 in proof of my ability to support him.
7. THAT I further confirm that during his stay and study period at the said Learn Key
Institute Malta he will have full access to funds and support wherever required.
8. THAT I swear this affidavit in support of TONY KIPROP application for student's visa and
any other relevant travel document to enable him travel and pursue studies in Malta.
9. THAT he has confirmed to me that he will come back to his motherland Kenya after
completing studies in Malta.
10. THAT I make this affidavit in solemn verification of the facts deponed to herein above
verily trusting all depositions to be true and correct to the best of my knowledge,
information and belief.
