MEDICAL REPORT
RE: IRE WA MIC HE AL OL OR UND ARE / MALE / 26 YEARS
This is to certify that the above named individual is a known patient of this facility. He was presented
to the cardiology unit of the hospital as a chronic case of Left Ventricular hypertrophy and Valvular
heart disease, a condition which developed as a result of the thickening of the left ventricle of his heart.

He presents with history of long lasting pain crises in the pericardium from the above ailment
subsequently accompanied with recurrent lightheadedness, fatigue, shortness of breath, abnormal
heart rhythm, murmur, increased blood pressure, peripheral edema, excruciating chest pain at intervals
of time in form of Angina, as well as sudden palpitations for which physical examination and clinical
diagnosis reveal the lady in generalized body distress.

Clinical diagnosis and test involving Electrocardiogram (ECG), MRI scan, Cardiac catheterization and
Cardiac CT scan was made and patient was placed on critical observation and medications following
findings. He had a relapse of severe pain crises recently with same symptoms and was treated
accordingly with the following medications: Tab Lisinopril, Anticoagulant (tab heparin), Statin and
Digoxin including few other IV injections administered.

However due to life threatening occurences, patient has been advised to avoid all strenuous
activities and will be appreciated if he is given preferential attention on basis of any request
pertaining leverage to stay close to the hospital for proper management and care.
