Instructions for Use


Before Submitting:

2. Use your correct account number and ATM card details
3. Add the current date
4. Keep copies for your records
Submission Options:

 In Person: Visit your post office branch with the original letter and
supporting documents
 By Post: Send via registered/speed post with acknowledgment due (AD)
 Email: Check if your post office accepts digital submissions
Documents to Attach:

 Copy of your savings account passbook (first page showing account


details)
 Copy of valid ID (Aadhaar, PAN, Voter ID, etc.)
 Original ATM card (if surrendering in person) or mention "Card will be
surrendered separately"
Follow-up:

Request a written confirmation slip or acknowledgment from the postmaster
