#CIRSEtakeover with Dr. Thiago Nunes Today’s case: a TIPS procedure performed in a patient with chronic thrombosis of the portal and splenic veins — a technically demanding scenario requiring experience and precise coordination. We had the pleasure of hosting Dr. Denis Szejnfeld, whose expertise in complex visceral interventions added tremendous value to the day. The procedure involved careful multidisciplinary planning, percutaneous access and recanalization, shunt creation, and portosystemic gradient optimization — all under real-time imaging guidance and hemodynamic monitoring. This case highlights how joining forces between experts is essential to overcoming complex challenges and delivering the best outcomes. 💪 #CIRSEsociety #IRinnovation #IReducation #InterventionalRadiology #TIPS #PortalVeinThrombosis #Teamwork #ExpertsTogether #SOBRICE #LatinAmericaIR SOBRICE - Sociedade Brasileira de Radiologia Intervencionista e Cirurgia Endovascular

Make sure to follow the European Conference on Interventional Oncology LinkedIn page to see this week's #ECIOtakeover with Dr. Rok Dezman from the University Medical Centre Ljubljana!
#ECIOtakeover with Dr. Rok Dezman UMC Ljubljana has major experience with electrochemotherapy (ECT) through close MDT teamwork and in partnership with the Institute of Oncology Ljubljana’s Department of Experimental Oncology, which helps develop electroporation-based therapies. ECT is a non-thermal local ablation: IV bleomycin plus brief electric pulses via needle electrodes. The pulses cause reversible electroporation, boosting drug uptake in tumor cells and producing targeted cell death in the treated volume (Image 1). 𝐏𝐨𝐭𝐞𝐧𝐭𝐢𝐚𝐥 𝐛𝐞𝐧𝐞𝐟𝐢𝐭𝐬 𝐨𝐟 𝐄𝐂𝐓: • Non-thermal (no heat-sink) • Vessel/duct-sparing • Effective in challenging locations • Intra-op or percutaneous delivery • Repeatable, combo-friendly • Vessel/duct friendly → safe for lesions near major hepatic vessels or bile ducts. • High local control in selected patients with complex or unresectable targets. 𝐖𝐡𝐞𝐫𝐞 𝐜𝐚𝐧 𝐄𝐂𝐓 𝐛𝐞 𝐮𝐬𝐞𝐝? 🤔 𝐈𝐧𝐭𝐫𝐚𝐨𝐩𝐞𝐫𝐚𝐭𝐢𝐯𝐞 𝐄𝐂𝐓 𝐟𝐨𝐫 𝐇𝐂𝐂: Prospective pilot data showed intraoperative ECT for HCC is feasible and safe, including centrally located lesions near major vessels, with high complete response rates. (Image 2, source: https://t.ly/m99vj) 𝐈𝐧𝐭𝐫𝐚𝐨𝐩𝐞𝐫𝐚𝐭𝐢𝐯𝐞 𝐄𝐂𝐓 𝐟𝐨𝐫 𝐜𝐨𝐥𝐨𝐫𝐞𝐜𝐭𝐚𝐥 𝐥𝐢𝐯𝐞𝐫 𝐦𝐞𝐭𝐚𝐬𝐭𝐚𝐬𝐞𝐬: In a prospective phase II cohort of unresectable CRLM, ECT achieved durable local tumor control and strong objective responses, especially valuable for central, vessel-adjacent metastases. (Image 3, source: https://t.ly/I_hNj) 𝐏𝐞𝐫𝐜𝐮𝐭𝐚𝐧𝐞𝐨𝐮𝐬 𝐢𝐦𝐚𝐠𝐞-𝐠𝐮𝐢𝐝𝐞𝐝 𝐄𝐂𝐓 𝐟𝐨𝐫 𝐇𝐂𝐂: We demonstrated that CT/US-guided percutaneous ECT for HCC is technically feasible, safe, and effective, expanding ECT beyond the operating room. (Image 4, source: https://t.ly/iJ31Q) 𝐋𝐨𝐨𝐤𝐢𝐧𝐠 𝐚𝐡𝐞𝐚𝐝: 𝐩𝐚𝐧𝐜𝐫𝐞𝐚𝐭𝐢𝐜 𝐜𝐚𝐧𝐜𝐞𝐫: In a porcine survival model, pancreatic ECT proved feasible and safe without pancreatitis or vascular injury, supporting exploration of ECT as a future option for locally advanced pancreatic cancer. (Image 5, source: https://t.ly/xh4GG ) ECT shows how IO is moving past one-size-fits-all ablation: a high-precision, non-thermal, vessel- and duct-respecting technique that expands what we can safely treat when heat-based approaches are not appropriate — while still aiming for strong local tumor control. ➡️ Also, check out this lecture from #ECIO2025 on electrochemotherapy for liver tumors by Prof. Franco Orsi in the CIRSE Library: https://t.ly/ISiG6 #interventionaloncology #electrochemotherapy #ECT #electroporation #HCC #colorectallivermets #pancreaticcancer #interventionalradiology
