As described in #8508, if OPcache is enabled, ext/mysqli can't be dl()'d; we work around that by loading the extension on startup.

@mvorisek, I'm working towards that PR. :) In this case there shouldn't be a problem, because extension_loaded() is called. If the extension is already loaded, and then dl()'d again, there is a crash on unloading. I'm checking that.
