PROCEDURE : ASSIST IN LIVER BIOPSY

PURPOSE : Confirm diagnosis of liver disease.

REQUIREMENT

1. Dressing set / liver biopsy set.


2. Liver biopsy needle.
3. Scalpel with sharp pointed blade.
4. Needle holder & suture.
5. Solution – iodine, spirit, opsite spray.
6. Lignocaine 1-2%.
7. Dressing towel.
8. Face mask.
9. Sterile gloves.
10.Sterile swab and gauze.
11.Syringe – 5ml &20ml.
12.Needle – 18G & 23G.
13.Elastoplast.
14.Scissors.
15.Specimen bottle with Nacl / formalin.
16.Lab request form.
17.Clinical waste.
18.Domestic waste.
19.Sharpbin
20.Protective sheet.

STEP

1. Verify Dr’s.
2. Greet patient and explain procedure.
3. Provide privacy.
4. Position patient in recumbent with right hand over the head.
5. Expose abdomen area.
6. Place protective sheet.
7. Wear face mask and open up set, pour solution, lay out sterile
items as required.
8. Prepare local anaesthesia.
9. Instruct patient to take deep breath and hold breath during
procedure.
10.Assist Dr in insertion of biopsy needle, aspiration and collection
of specimen.
11.Apply and secure dressing.
12.Position patient in right lateral for 1-2 hours.
13.Make patient comfortable.
14.Instruct patient on CRIB for 6 hours.
15.Label and send specimen to lab as soon as possible.
16.Clean and clear equipments.
17.Record and report result.

NURSING RESPONSIBILITIES

1. Reassure client to gain cooperation.


2. Ensure consent is obtained.
3. Emsure procedure is carried out on correct client at correct site.
4. Check vital sign for pre, intra and post procedure to detect any
complications.
5. Perform procedure in a correct manner to prevent complications.
– Pain, bleeding, haematoma, hypovolemic shock,
pneumothorax, haemothorax,subcutaneous emphysema and
infection.
1. Observe puncture site for bleeding, haematoma, swelling and
tenderness.
2. Ensure client’s comfort is maintained throughout procedure.
– Right lateral position post procedure, CRIB and pain free.
1. Send labeled specimens in appropriate container with lab request
form.
2. Record findings and report abnormalities.
