MAJOR DEPRESSIVE DISORDER


2. Age
Any age but commonly seen in 40y/o
3. Sex
More common in female (2:1)
4. What hypothesis support it
1. Monoamine hypothesis
- Depletion of monoamine such as 5HT and NA
2. Endocrine abnormalities
- Hyposecretion of cortisol and TSH
5. Clinical features

3. Genetic : 41%:13% (monozygotic: dizygotic) concordance


1. Depressed mood (children irritability)
2. Loss of interest or pleasure in all activities (ahedonia)
3. Significant loss/ increase appetite or increase/ decrease body weight
4. Insomnia or hypersomnia
5. Low energy of fatigue
6. Psychomotor agitation or retardation
7. Poor concentration or indecisiveness
8. Feeling of worthlessness, guilty, excessive
- no one can help, no future, burden of others, dislike by others

9. Recurrent though of death, suicidal ideation or attempt suicide


Other Ammenorrhoea, constipation, decreased sexual drive
6. Insomnia

When cannot sleep wake up and do housework = bipolar(energetic)


To rule out melancholic
Taking stimulant, caffeine, vigorous exercise before sleep ?
What he do when cannot sleep ?
Usual sleeping pattern

8. To exclude

1. Due to substances, medication or other medical condition


2. No prior [endogenous] episodes of mania or hypomania*
3. No schizophrenia, schizoaffective or schizophreniform
4. Adjustment disorder
- Maladaptive response to an identified streesor

5. Dysthymia
- Depression for more than 2 years with symptoms free period less than 2mth
9. Diagnostic criteria

5 or more symptoms in 2 weeks


Change from previous function (social, occupation)
At least one is depressed mood or loss of interest/pleasure

10.Meaning of significant weight


loss

Change in body weight for more than 5% in a month


In children consider failed to achieve expected weight

11.Meaning of recurrent

An interval of 2 consecutive months where there is no major depressive


episode in between
