Pico Presentation

The document summarizes two research articles about the effects of hourly rounding on falls in hospitals. Article one found that hourly rounding significantly reduced patient call light use. Article two found that hourly rounding reduced the monthly average of call bells and falls. Both articles supported the hypothesis that hourly rounding improves patient safety and satisfaction by reducing call lights and falls, though the study designs were quasi-experimental. The findings suggest hourly rounding could change hospital policies and standards of care, but workload may be a barrier to implementation.

In hospitals, what are the

effects of hourly rounding on

the incidence of falls?
The purpose of this presentation is to apply the skills we have gained
throughout the course of this class and answer a PICO question by reviewing
two research articles.

Why these articles?

Article one
Call lights
Falls
Patient satisfaction

Article two
Call lights
Falls

Article One
Theory
This study sought to determine the frequency of and reasons for patients call light use, the
effects of one-hour and two-hour nursing rounds on patients use of the call light, and the effects
of such rounding on patient satisfaction, as well as patient safety as measured by the rate of
patient falls.

Type of study
analytic study

Type of design
quasi-experimental nonequivalent groups design

Methods
A six-week nationwide study

Results
Binomial test revealed significant reduction (P= 0.007) in call light use for the one hour rounding.

Article two
Theory
The premise of the current study is that patients would perceive e that proactive nurses who provide
consistent care will meet their physical and emotional needs. Specially, when nursing rounds on stroke
unit, conducted on a regular schedule by nursing staff that perform a specific set of actions, would;
reduce call light use, increase patient satisfaction, improve patient safety as measured by the frequency
of patient falls and finally decrease patient bed sores.

Type of study
analytical

Type of design
quasi experimental non equivalent groups design

Methods
8 week study

Results
after hourly rounding by nurses the overall monthly average of call bells were reduced for
different reasons.

Ethics
Informed consent
No harm

Article one: strength, quality

credibility
Contribution to EBP
Yes, hourly rounding, more safety less call lights

Fist hypothesis was supported.

Analysis is weak, two reasons
Patient fall was not as significant in the two hour rounding
Quasi experimental design
Generalizable to majority of U.S. hospitals

Article two: strength, quality

credibility
Hypotheses was supported
Analysis weak
Quasi experimental design
Contribution to EBP
Yes, reduction of falls

Article one: Are findings relevant

to practice.
Levels
Individual, policy, standard of care

Implications for practice change

Potential barriers
Work load

Could unit secretaries answering call lights help to improve patient moral and
safety?

Article two: Are findings relevant

to practice.
Levels
Individual, policy, standard of care

Implications for practice change

Potential barriers
Work load
Type of unit

Can different types of units impact the significance of hourly rounding?

Conclusion
In conclusion, the results from the two articles show that hourly rounding can
improve patient safety, decrease the amount of call light usage, and improve
patient satisfaction.
