This is to certify that I am allowing my daughter Louie Marie M. Dadat to go on a practicum for a
minimum of 300 hours starting on June 26, 2013 at Geiser Maclang Network in partial fulfilment
of the requirement for the degree in Bachelor of Science in Business Administration.
It is understood that she abide by the rules and regulations that may be imposed by the
Supervisor for her welfare and safety.
I fully agree to waive any responsibility on the part of Geiser Maclang and/or the representative/s,
in case of any untoward incident that may happen to my daughter during the duration of the
practicum.
