ACKNOWLEDGEMENT

We would like to express our deepest gratitude first to our Lord God Jesus
Christ and to all the persons around us who extended supports for the
accomplishment of this document.
To our parents, thank you for your love, never ending support, motivation
and inspiration you have shared with us. For the accomplishment of this study,
you are the person who are always there to give their best for their childs dream.
To our clinical instructor, Mrs. Rosalie Cabritit who guided us during the
CHN duty in Salyan. It is such a wonderful thing that there is always someone
around who is always ready to share knowledge and wisdom for our own
improvement.
To the community and residence of Salyan, Abucay, Bataan for the warm
welcome and for allowing us to be a part of your community. To each families for
the cooperation and openness. This study wouldnt be possible if it werent for
them.
To our classmates and friends, GRACIAS! for sharing ideas, effort and
