Medical Nutrition Therapy Diet


1. Purpose
a. Nutrition Indicators: Client is experiencing gastrointestinal discomfort after consuming
gluten containing products. The clients current 24hr recall is as follows: Breakfast- One glass of
orange juice, two piece wheat toast, eggs, and bacon, Lunch- one glass of milk, small personal
pan pizza and a bag of lays potato chips, Dinner- Spaghetti with meatballs and marinara sauce,
garlic bread and water.
b. Criteria to Assign the Diet: Client needs to be put on a gluten free diet that restricts
wheat, barley, etc. To help reduce inflammation in the intestines and help heal micro villi that has
been damaged by consuming gluten.
c. Rationale for Diet: The client has been diagnosed with celiac disease from prior tests
done through AGA, EMA, and tTG antibodies that are used in serological testing to diagnose
celiac disease. Client has tested positive for celiac disease and celiac disease is a gluten
intolerance so removing gluten from clients diet will lead to lowering inflammation in the
intestines and reduce the symptoms and complications that are being experienced. This will help
the micro villi that are within the intestines heal.

2. Population
a. Overview: An estimated 1 out of 133 Americans suffers from celiac disease which adds
up to about 3 million people in the United States.
b. Disease Process: Celiac disease is an autoimmune disease caused by the ingestion of
gluten. Celiac disease starts to become present as the micro villi in the intestines starts to get
damaged. This results in symptoms like diarrhea, vomiting, stomach bloat, FTT, and stomach
pain.

c. Biochemical and Nutrient Needs: Nutrient needs stay the same for someone with celiac
disease except they are not allowed to ingest gluten. Products can be found to fit the
carbohydrate and grain need that have had gluten removed in them. Nutrient needs such as a
higher protein diet may be needed if the client has a lot of damage to the intestines that will need
to be healed and the higher protein intake will help in the process of healing the intestines.

3. General Guidelines
a. Nutrition Rx: Gluten Free Diet
b. Adequacy of Nutrition Rx: It will reduce the symptoms and complications associated
with celiac disease.
c. Goals: Have client be on a gluten free diet to help get rid of the diarrhea, vomiting,
stomach bloat, and gastrointestinal discomfort.
d. Does it Meet DRI: Yes, by consuming different foods that do not contain gluten it will
still meet the DRI.

4. Education Material
a. Nutrition Therapy: Teaching client on what products contain gluten such as:

Pastas: raviolis, dumplings, couscous, and gnocchi.


Noodles: ramen, udon, soba (those made with only a percentage of buckwheat flour)
chow Mein, and egg noodles.
Breads and Pastries

Crackers

Baked Goods

Cereal & Granola bars

Breading & Coating Mixes

Informing client on different products that make gluten free foods that they can eat and explain
why they are on a gluten free diet and how it is going to help them not have the symptoms that
they have been experiencing.

b. Ideas for Compliance: Explaining to client about all the gluten free options there are and how
there are different gluten free pastas and breads so they can still have all of their favorite foods
just they have to be made without gluten. Letting the client know that they will have fewer
complications and not have to deal with diarrhea, vomiting and stomach pain.

5. Sample Menu
a. Foods Recommended: Corn in all forms (corn flour, corn meal, grits, etc.). Plain rice in
all forms (white, brown, wild, basmati, enriched rice, etc.). Amaranth,
arrowroot, buckwheat (kasha), cassava, flax, millet, quinoa, sorghum,soy, tapioca and teff.
Flours made from gluten-free grain, nuts, beans and coconut (Gluten-Free,2016).
