All returned units must have the RMA number clearly indicated on the box and a paper copy of this form included in the shipment. The "bill to"
information is required if different from the shipment address. Should the Customer fail to return the defective unit within thirty (30) days from the
receipt of the replacement unit, or if a defective unit sent by the Customer is determined to be a "No Trouble Found Unit" or if unit is damaged in
shipment due to improper packaging; Customer agrees that Fortinet shall be authorized to invoice Customer at the current list price of the product sent
to replace Defective Unit.
Please use original shipping boxes and packing or alternatively use adequate packaging to ensure the returned product is not damaged
during shipment.
