I. INTRODUCTION
This case is presented by Group 5 students from Arellano
University (Pasay), 3rd year level block 2. This case study is all about Urinary Tract
Infection (UTI).
UTI defines a condition in which the urinary tract is infected
with pathogens causing inflammation. There is a consensus that most pathogenic
microorganism such as Escherichia Coli the colon, the perineal region and in the
female in troitus vaginal and the periurethral region. Facultative the may further
ascend to the bladder and/or the kidneys. If structure of the urinary tract are invaded,
accurate diagnosis and treatment are necessary in order to ensure optimal
management to prevent further complications. UTI results from the interaction
between urophathogens and the host. The microorganisms may have particular
urophathogenic properties, explaining the occurrence of infection in an otherwise
normal urinary tract. On the other hand, non – urophathogenic can induce acute
infection in the presence of urological abnormalities, or when the host’s defense
mechanisms are impaired: in pediatric patient and old age, pregnancy diabetes, and in
the immuno compromised patient including renal transplant recipients.
The chosen client is a 54 years old, female that is admitted
because of fever. The initial diagnosis is UTI. Later on the sepsis is ruled out by the
attending physicians. This study is entitled the whole course in the ward of the client
from_______________. The pathophysiology will give us view on how UTI is
acquired and what are its signs and symptoms. This study also tends to give health
teaching on how to prevent the development of the UTI.
The presenter’s hope that this presentation; though it has its
limitations, such as the care given and the communication barriers between the
presenters and the informant, will give our fellow students added knowledge on UTI.
II. DEMOGRAPHIC DATA:
In this course of this paper the name given to hide the patient’s
identity is Mrs. Barts (as they requested). Mrs. Barts is a 54 years old female resides
at Novaliches, Quezon City. In this study our informant is the daughter – in – law of
Mrs. Barts and we will hide her identity by calling her Mommy Pancake. Mrs. Barts
was admitted last July 7, 2007 with chief complaint of fever the initial diagnosis is
Acute Gastro Enteritis (AGE) and the final diagnosis of UTI. She was not yet been
discharge until we had our final rotation from the hospital.
III. CHIEF COMPLAINT: Fever
IV. HISTORY OF PRESENT ILLNESS
3 days prior to admission, the patient has an intermittent fever,
documented at 39 – 40°C. Patient was self medication with paracetamol which
provided temporary lysis of fever. Still with occurrence of fever, consult was done at
Novaliches District Hospital and was admitted.
V. PAST MEDICAL HISTORY
Mommy Pancake states that her mother – in – law, Mrs. Barts
lives with her, though she was separated with her husband for 9 years. She said that
Mrs. Bart also live with her strange husband for a month but later on she go back with
her because Mrs. Barts doesn’t get along well with her son’s live in partner. Mrs. Bart
has an history of Hypertension, and whenever she was emotionally stress her blood
pressure elevated and her diet was not monitored that why Mrs. Barts decided to go
back with mommy pancake her favorite daughter – in – law. Mrs. Bart is a very
energetic Lola as mommy pancake said she used to help her in taking care of her
children while mommy pancake was away from work. Mrs. Bart doesn’t have any
problem except her hypertension. But mommy pancake stated that her mother – in –
law has her maintaining medications for that.
