Dear Prospective Parents:

Thank you for requesting an application packet. I am enclosing the forms that
need to be returned to me in order to begin the application process for the
2011/2012 academic year.

Please fill out:

 application form (one per student)


 parent interview form (one per family)
 student interview form (grades 7–12, one per student)

Please ask your current pastor to fill out the following forms and mail or fax
to the school.

 Pastoral recommendation form (one per family)


 Pastoral recommendation form for secondary students (7–12, one per
student)

Return (or mail) these forms with a $100 per student application fee to my office
($125 enrollment fee for Pre-K students).

Please bring report cards or transcripts for the last one-to-two years when you
come for testing. First- through twelfth-grade students will take a portion of the
Stanford Achievement Test, covering math computation and reading
comprehension; kindergartners will take the DRS-R2 (Developmental Readiness
Scale Revised Second Edition).
