The Edward and Eva Underwood Charitable Foundation scholarshipis awarded to students to
provide educational opportunities to them and to enable them to pursue their studies. To be
consideredfor scholarshipsupport,all applicantsmust meet the following criteria:

r Philippine,Frenchor Canadiannationality
r must be able to demonstratefinancial need
o proof of over-averagestudy record

In order to qualify for the scholarshipstudentsmust send in a completed application form with
supporlingdocumentation.Original materialswill not be returned.

A1l applicantsmust authorize the releaseof any information relating to their application to the
Edward and Eva Underwood Charitable Foundation. Information may be requestedfrom high
school, college, university, govemment, or community sources, and may include personal
evaluationsandtranscripts.

Scholarshipsare awarded to cover tuition, fees, and other school related expenses.Scholarship
accountscan be createdat the educationalinstitution at which the recipientis enrolled.
Applications are evaluatedbasedupon the applicant's demonstratedfinancial need, career goals,
academic and/or volunteer record. Financial documentationmay be required to demonstrate
financial need.

Recommending institution: Please ensure that the following documents are enclosed: Proof of
citizenship, convincing documents to approve financial need, Academic Trunscripts, copv of
Letter of Acceptance from institution (if confirmedl and two Referee's Report Forms (enclosed)
snd return completed application form as soon us possible to the following address:
