IMPORTANT TICKET INFORMATION
Tickets may be purchased during the weekend rehearsals between
1:30-3:30 or online at PDMProductionsva.com website.
Tickets may also be purchased at the Dress Rehearsal at Deep Run High
School, Friday, December 7, beginning at 4:30pm, and at the door on the
day of the shows.
Tickets can also be held at WILL CALL to be picked up at the door.

I, of my own free will, am participating in a program offered by PDM Productions, Inc. (Pilates Dance
and More Productions,Inc.) in the Performing or Fine Arts. I understand that participation in the
Performing and Fine Arts related programs, like any activity, may present some unavoidable risk of
injury; especially to people who have pre-existing conditions, illnesses, or medical disabilities. I
understand that the use of various types of instruments, apparatuses, equipment, props, etc., also carries
with it a risk of injury. I recognize that many changes may occur as a result of participation in any of
the activities, rehearsals, workshops, or performances; including possible short-term aggravation of
some symptoms, feeling of tiredness, light-headedness, increased energy, mood changes, etc.
I also understand that a medical evaluation is advisable before commencing any program of physical or
mental development. I have and will continue to keep PDM Productions,Inc., its owners, employees,
volunteers, and contractors, fully informed of any physical or mental condition or disability which
would prevent or limit my participation in any of the activities in which I am engaged. I acknowledge
that opportunities in the performing and fine arts may have substantial physical and mental benefits. I
also acknowledge that neither PDM Productions,Inc. ( Pilates Dance and More Productions,Inc.) ,its
officers, employees, or volunteers, are engaged in the diagnosis or treatment of any medical disease or
deficiency , (whether physical, mental, or emotional).
I expressly assume all risks of my participation in any of the programs offered by PDM Productions,Inc.
( Pilates Dance and More Productions, Inc.) and waive any claim which I might otherwise bring against
Pilates Dance and More Productions,Inc. doing business as PDM Productions, Inc. ( Pilates Dance and

Information on Nutcracker Photography
On Friday, November 9 , we will be having a photo-shoot for all cast
members for inclusion in the Souvenir Program. Each dancer will be
photographed in full costume and will be identified by name in the book.
Please sign the photo release form (below) in order for your child to
participate.
In addition to photographs taken for the Souvenir Program, we will be
taking pictures of your child during rehearsals. These photographs may be
used for promotional materials, such as articles in print, The Nutcracker
Sweet Facebook page, or other advertising. We also have the option of
putting some behind the scenes photography on our Performance DVDs
this year. If you would be willing to allow your childs photograph to be
included in these materials, please sign the release form below.

Nutcracker Sweet
Photo-Shoot and Souvenir Program Information
Photo Shoot Information
The Photo-Shoot will take place on Friday November 9. Although the tentative
schedule for the photo-shoot is not available yet, we hope to be finished by
10:00pm. Younger dancers will be scheduled toward the beginning of the night.
Individual Photos will be offered this year! We will have a sign-up sheet
available for those interested in individual shots. More information to come!
All female dancers should wear a nude leotard, as everyone will be
changing into costumes in an open studio. Please have your dancer wear
light make-up and have hair done upon arrival at the studio.
Souvenir Program Information
As part of the production fees, all dancers will receive one complimentary copy of
the Souvenir Program. If you would like any additional copies you need to pre-
order. Please use the order form below. All souvenir books will be available for
pickup the day of the first show.
All Souvenir Program Orders are due by November 17, 2012.

Performance DVD Information Sheet
Our Videographer This Year!
Bruce Berryhill, Award-winning Television Producer/Director/Videographer
In addition to extensive technical experience, Mr. Berryhill is a former dancer, husband
of a dance professional, and father of dancers. As such, he offers both an artists and a
parents perspective on dance videography. His special expertise has qualified him to
film dance productions for Richmond Ballet, Sun King Dance, and others. His passion is
creating memories for parents and families.
Mr. Berryhill will be filming both performances this year. As an additional benefit to
parents, Mr. Berryhill will be creating a special photographic scrapbook set to music,
utilizing behind the scenes rehearsal photographs.
DVD Descriptions:
Saturday and Sunday Performances, shot wide (but as close as possible) with all
performers onstage in every shot. This DVD will include still photos at the end (Photo
Scrapbook).
(The Photo Scrapbook will include still photographs set to classical music. If there are
a sufficient number of photographs submitted, this may be provided on a separate
(third) DVD, at no extra charge.)

How to Help Promote The Nutcracker Sweet
Here are some ideas on how to help promote
The Nutcracker Sweet this year.
A pdf file containing this years Nutcracker Sweet poster will
be posted on the Nutcracker Sweet website. You may
download this poster on your home computer, print it out
and distribute copies to friends, co-workers, etc., as needed.
Full-size posters (16X12) will be available in October for you
to take to places of business and other venues to display on
community events bulletin boards or store windows. Some
places that may display our posters include: libraries,
schools, churches, grocery stores, restaurants, dance
goods stores, and coffee shops. Please distribute the
posters to help with ticket sales.
If you have a Facebook account, visit The Nutcracker Sweet
Facebook page. Click like and encourage others to do the
same. Also, visit The Nutcracker Sweet event sites on
Facebook. Please share these with others.
If you are on Twitter, please follow Kate Semp @ketilave as
she will be Tweeting about these events.
Tickets will be available for purchase at the studio during
rehearsals in October. You may use either of the enclosed
ticket order forms to place orders. (The spreadsheet works
best for larger numbers of orders.)
We are asking each student to sell a total number of at least
10 tickets for the run of the show. Please do your part in
helping with the ticket sales. Ask neighbors, friends, school
friends, co-workers, teachers, to come see this wonderful
production of The Nutcracker Sweet.
2012 Nutcracker Sweet - Ticket Information
Ticket Cost: $15.00

Anyone seated in the auditorium MUST have a ticket. Seating is General Admission. Doors
will open 30 minutes prior to each performance. All performers can be picked up at the Sweets
Party at the conclusion of the show.
