1) PLAAFP: DH can read 96 WPM with 0 errors on a 4th grade level.

He is on track to meet his goal of 108 WPM at a 4th grade level. Reading fluency goal - When given a passage at the 5th grade reading level in class, DH will read at 120 words per minute with 95% accuracy, using the appropriate rhythm, volume, and flow. Obj. 1: When given a passage at the 5th grade reading level, DH will read at 90 words per minute with 95% accuracy, using the appropriate rhythm, volume, and flow. Obj. 2: When given a passage at the 5th grade reading level, DH will read at 105 words per minute with 95% accuracy, using the appropriate rhythm, volume, and flow. 2) PLAAFP: DH can get 17 correct responses with 2 errors (89% accuracy) on a 5th grade MAZE test. He is on track to meet his goal. DH can answer text based or factual comprehension questions with 75% accuracy. Reading comprehension goal After reading a passage at the 5th grade reading level, DH will answer verbally or in writing predictive and inferential questions about the text with at least an 80% accuracy. (4 out of 5 correct answers.) Obj. 1: After reading a 5th grade reading passage, DH will retell verbally or in writing, the passage, including 5 facts or details presented in correct chronological order with 100% accuracy. 3) PLAAFP: On average, DH can write a 5-sentence paragraph of 53 words with 6 errors. (89% accuracy) DHs Correct Writing Sequences score on the initial story starter falls in the average range compared to 5th grade student norms. Writing goal Given a topic, DH will write a 3 paragraph essay on a single topic in a logical fashion with at least 90% accuracy. Obj. 1: DH will be able to write a 5-sentence paragraph on a single topic, using grade-level appropriate grammar, spelling, and punctuation, with 100% accuracy. 4) PLAAFP: As of 3/8/13, DH could add and subtract multi-digit problems using regrouping with more than 90% accuracy. He could perform single digit multiplication problems with 100% accuracy. He could not add fractions. (0% accuracy) Math calculation goal DH will be able to multiply multi-digit numbers using a variety of strategies. Obj. 1: DH will be able to multiply single digit numbers with 100% accuracy.

Obj. 2: DH will be able to multiply multiple digit numbers using regrouping and representations, e.g., that 20*9 means 20 groups of 9. Obj. 3: DH will be able to multiply multiple digit numbers using taught heuristics and regrouping strategies. 5) Math concepts and application PLAAFP: DH can solve a word problem at the 4th grade level with 33% accuracy, and can identify the steps by which he attempted to solve it with 47% accuracy. Goal: During assessments in his math class, Hume will be able to solve a math word problem at the 5th grade level and to identify and describe the process by which he solved it. He will be able to complete this objective with 90% accuracy. Obj. 1: During assessments in his math class, Hume will be able to solve a math word problem at the 4th grade level and to identify and describe the process by which he solved it. He will be able to complete this objective with 90% accuracy.
