turned in as a whole (no partial weeks turned in separately for payment in different pay periods).

INSTRUCTIONS FOR ALL STAFF:
The hours per day worked and the days of the week worked are scheduled by the HIMS
Operations Manager or local HIMS manager. You must contact your manager prior to the
beginning of your work schedule if you will not be on time or will be unable to work. All work
schedules in excess of 40 hours per week must be approved in advance by the HIMS Manager.

Record only hours worked at the client's facility. Do not include travel time, meal periods of 30
minutes or more, or break periods of 20 minutes or more. Enter time as it would appear on a clock
(8:00 am, for example). How to use the boxes under the "Time Worked (Enter Time of Day)" area:

Time In - Time when you report for duty at the client's facility.
Time Out - Time you stop working to take your first break, lunch, etc.
Time In - Time you return from that first break, lunch, etc.
Time Out - Time you stop working for your second break, etc.
Time In - Time you return from that second break, etc.
and so on, until your last Time Out when you leave work for the day.

There are four sets of time in/time out for each day. If you find you need more, please ask for
assistance modifying your time record to allow for additional INs and OUTs.

This sheet may also be printed and hand-written. If you fill out your time record by hand, please
be sure to check your time calculations.

BI-WEEKLY TIMESHEET INSTRUCTIONS FOR EXCEL USERS:

You only need to enter one date for the two-week period (Week One's Monday date, in the red
font) and the rest will fill in automatically. The start date for the next two-week period will
always be the same as the due date for the last period. Please enter the date in MM/DD/YY format.

The totals calculate automatically. Please be careful not to eliminate or change the formulas
in the yellow total column. If you do accidentally eliminate them, please ask for assistance
(contact info below).

If "#VALUE!" or "######" appears in the WORK TOTAL column, a time has been entered in a way
that Excel cannot recognize. Please review the times for that date and re-enter as shown above.
Though the samples above show quarter-hours, other times are accepted by Excel.

Once you get used to it, it's not difficult at all to use the Excel weekly sheets. If you have any questions,
