DESCRIPCION OF THE CARGO


Machinnery for Make Blyster Packs of Pills Brand MARCHESSINI, Model MST 200
Package: Two Wood Crates with the following dimensions and weight:
BOX 1:
Length 3,06 m x Wide 1,52 m x High 2,31 m
BOX 2:
Length 1,20 m x Wide 0,80 m x High 0,80 m
Both boxes together have a weight of 2000 kilograms.
Cargo was Airborne from Mexico City to Panama City by DHL Global Forwarding with MAWB 810 83389725
Shipped by: GLAXO SMITH KLINE SA DE CV at San Lorenzo HUIPULCO TLALPAN, Calzada Mexico,
Xochimilco 4900 DF 14370 Mexico.
Consignee to: GLAXO SMITH KLINE PANAMA S.A. at Urbanizacin Industrial Juan Daz entre calles A y B
con Apartado Postal 0819 0853.
Paying and Air freight total of: USD10.005,15 due to DHL GLOBAL FORWARDING. (Please attached copy).

NARRATIVE OF SURVEYOR OBSERVATIONS


We arrive at 15:00 hours of November 07th 2016 at the PLANTA GSK in Juan Diaz. We were received by Mr.
Facundo Manuel Paz as GSK representative. Immediately we were conducted to the Receiving area of
BODEGA DE MATERIALES (Pre-Storage of Materials Area), were we could observe both Boxes above
described. We inform also that we had been representing to MULTI CARGO CONTROL, s.a. here.
We had interviewed to Mr. Facundo Paz and he has shown us a Picture in his cellular phone were could be
observed a Box, Apparently the same one that we are now surveying, loaded on top a platform truck with no
Close compartment and under raining conditions. That was his major claim about the services of our
principals. His complains were basically for lack of care in the transportation and mobilization of the cargo
and protection against the rain.
We Also had received verbal claims of the small damage present in the package that the GSK representative
argue was caused by the handling of the cargo for DHL GLOBAL FORWARDING, but he not provide any
confirmation of such allegations.
After conclude the Interview to Mr. Facundo Paz, we had proceed to make visual inspection of both boxes
for all possible and accessible sides. In the Box Number 2 we had been able to see in the interior of the box
and had observed the cargo inside it.
We had taken several pictures all around the boxes and take note of our observations and them we had risen
our factices observation of the condition of the boxes.

Forewords

Our Company, Marine Multiservices s.a., has been contracted by MULTI CARGO CONTROL S.A. to perform
an independent assessment of the cargo condition of a cargo transported by DHL GLOBAL FORWARDING
for GLAXO SMITH KLINE de Panama.
The following survey report only covers the aspects to which we had direct access at sight or by means of
DOCUMENTS review and Internet searching of relevant data.
To the best of the undersigned surveyors knowledge and experience, the information contained in this
report is totally true and has not omitted or withheld any information. Therefore the undersigned surveyor
will not be liable for alleged mistakes or omissions exceeding the scope of the foregoing.
In accepting this report, it is agreed that the extent of the obligation of this firm in respect thereto is
limited to furnishing a surveyor believed to be competent, and in the making of this report, the surveyor
is acting on behalf of the person, company or firm requesting the same and no liability will fall upon this
firm or surveyor for the accuracy, errors and omissions thereof. The information herein contained is only
the one that has come to the attention of the surveyor.
The present survey report has been performed and signed without any kind of prejudices, under the most
strictly ethics and professional standards of surveying.

OBSERVATION OF CARGO CONDITIONS


We had found the following conditions in the package of the cargo. (cargo was not accessible because the
crated were closed)
Box 1:
Plastic film surrounding the box was found tor in several parts, letting bare the wood.
One steel band was broken
The wood pallet under the box has several plank broken, apparently caused by the wrong collocation of a
fork lift that could case this minor damage.
The wood in one of the upper corner of the box has a small piece torn.
None of the above damage reported has exposed the cargo to the elements and the integrity of the box is not
compromised at all. The wood of the box hasnt any mark of had been received water o any other humidity
condition.
Box 2:
This Box has the half of the upper cover removed and the nails were exposed. According with Mr. Facundo
PAZ, the reason is because the Custom personnel had open it for confirmation of the cargo description. We
had been able to lift the wood cover and observe and take some pictures of the cargo inside. The cargo had
any sing of damage in its wrapping or in its structure at all. In the opinion of this surveyor there is no damage
present in the cargo inside the box Number 2.
The crate box integrity is intact and no plastic film in the top was observed, only a wrapping around the side
of the box was observed.
No humidity o waters marks was observed in any part of the box Number 2.
No further observation was made in this inspection.
