Locker Rental Agreement


TERMS AND CONDITIONS:
1. I understand that my locker assignment will be automatically renewed, and I
will be charged the annual locker fee of $40 after the expiration date listed
above if I do not inform the front desk staff of my decision to cancel my
locker assignment.
2. I understand that if I fail to pay my annual locker fee within 60 days of the
charge or if I become inactive for more than 6 months, The Wellness Center
reserves the right to remove contents of the locker. If the contents are not
picked up from the front desk within 30 days after removal, The Wellness
Center reserves the right to donate the retrieved items to Goodwill.
3. I understand that I am responsible for securing my personal belongings with
either a personal lock or with the lock provided to me.
4. I understand that The Wellness Center will not be held responsible for lost,
damaged, and/or stolen property from this locker.
5. The Wellness Center reserves the right to open and inspect lockers without
the consent of the member to whom the locker is assigned to in instances
where locker procedures are being abused or in the case of an emergency.
