REQUIREMENTS FOR FIDELITY BOND

1. Duplicate copies of General Form No. 57A (Request Form), No. 58A (Application
Form)
- Notarized
- 2 passport size ID

2. Application form shall be accompanied by:

- Latest SALN
- List of Bondable Officers ( Annex D)
- Certification of Good Moral from LCE
- Court Clearance
- Certification from MLGOO for election/appointment

To be filed at Bureau of Treasury at Rizal Ave., Puerto Princesa City.

REQUIREMENTS FOR LAND BANK FOR NEW SIGNATORY

1. Fidelity Bond (At least submitted to Bureau of Treasury)


2. SB Resolution (Notarized)
A resolution submitting to Land Bank of the Philippines the new
authorized signatories of the barangay accounts indicating the name of
Punong Barangay and Barangay Treasurer and their predecessor if newly
elected or appointed.
3. Oath of Office (PB and BT)
4. Certification of DILG (MLGOO) as newly elected PB/ Appointment of BT
5. 2 pcs. 2x2 ID
6. Copy of valid ID

To be filed at Land Bank of the Philippines - Puerto Princesa City. Please proceed
to new accounts section.
Note: Please bring all other important documents.
