Mangao raised Arcega alone after the father left when she was five years old. AFFIDAVIT OF PARENTAL ADVICE

I, GINAFIE L. MANGAO, 49 years old, married and a resident of P-5 Bacood,


Brgy. Maug, Butuan City, Agusan Del Norte, after having been duly sworn in
accordance with law, depose and say.

1. That I am the mother of JAYLENN M. ARCEGA, 22 years old and a resident


of P-5 Bacood, Brgy. Maug, Butuan City, who was born out of wedlock, on
October 10, 1993;
2. That I raised my child, Jaylenn alone as his father left us in February 1998
when Jaylenn was only five (5) years old;
3. That in October 4, 2003, I married to my present husband, Edelberto S.
Erno, with whom I bore another daughter;
4. That at present, my daughter JAYLENN M. ARCEGA, is applying for a
Marriage License before the Office of the City Civil Registrar of Butuan City,
and she is being required to submit a parental consent/advice pursuant to
the requirement of the Family Code of the Philippines;
5. That only me can give such parental consent/advice since Jaylenn’s father is
missing and his whereabouts are unknown, and I am now happily married
to another person;
6. That I execute this affidavit to show proof of the foregoing fact in support
of her application for the issuance of a marriage license.

FURTHER AFFIANT SAYS NOT.

IN WITNESS WHEREOF, we here unto sign this 27th day of July 2016 at the
City of Butuan, Philippines.
