AFFIDAVIT OF EMPLOYMENT

I, MA. RUTH ELTANAL PALBAN, of legal age, single and a resident


of Zone-A Bougainvilla Puti Villaverde, Iligan City, after having been duly
sworn to in accordance with law hereby depose and say:

1.)That I have been working at Medina General Hospital, Ozamiz City


for four (4) years now;

2.) That I was assigned as staff nurse in the ward station (medical
surgical ward, Ortho ward, OB- Gyne Ward and Pediatric Ward, on
rotation basis) from March 29, 2010 up to September 30, 2011;

3.)That I have been assigned as a staff nurse in the operating room of


the same hospital since October 1, 2011 up to the present;

4.)That I was issued reference certifications by the Director of Nursing


Service Department and OR/DR/NICU Nurse Supervisor of the said
hospital as reference for employment abroad;

5.)That I am executing this affidavit to attest to the truth of the


foregoing facts and for whatever legal purpose this may serve me.

IN WITNESS WHEREOF, we have hereunto set our hands this 28th


day of March 2014 at Ozamiz City, Philippines.
