I.

ADVANCE CARDIAC LIFE SUPPORT MEDS


Epinephrine 1 mg/amp 1 amp IV q3-5 minutes
OR Epinephrine 2-2.5 mg in 10 cc H2O via ET tubes q3-5 minutes

Anti-arrhythmics:
Vfib or Vtach
Amiodarone 150-300 mg (1-2 amps) slow IV push in 10 minutes, then 1mg/min for 6 hours, or
Lidocaine 50-100mg (1 mg/kg) IV, then drip at 2mg/min up to max of 4mg/min

Torsades de pointes/polymorphic Vtach


Magnesium sulfate 1-2 grams IV in 10 cc D5W given in 5-20 minutes
If with pulse, MgSO4 1-2 grams IV in 50-100 cc D5W

Vtach with pulse


Synchronized cardioversion (100, 200, 300 J) + sedation, OR
Amiodarone 150 mg/amp 1 amp slow IV push in 10 minutes, then drip 1 mg/min in 6 hours

Bradycardia
Atropine 0.5-1 mg IV (0.04mg/kg; max 4mg)
May give Dopamine or Epinephrine drip as temporizing measure

II. HYPERTENSIVE URGENCY/EMERGENCY

Nicardipine drip

Nitroglycerine drip

Clonidine 75 mg SL, up to 3 doses

III. SEPTIC SHOCK


Dopamine drip 200mg/amp 1 amp + 250 cc D5W to run for __ cc/hr (same as ugtt/min)
Titrate by 2-3 cc/hr to maintain BP>= 90/60

Midazolam 5mg/mL amp (0.07-0.1mg/kg) IM or IV

Hydrocortisone IV

Nalbuphine 10 mg/mL amp (0.15-0.20 mg/kg IV q3-6hrs)
Tramadol 50-100 mg IV or IM q6-8hrs (max 400mg/day)

Celecoxib 200 mg tab BID


Naproxen 275mg tab TID

XI. CAP

Cefuroxime 750 mg IV q8hrs ANST ()


Azithromycin 500 mg tab OD x 3 days
