AFFIDAVIT OF LOSS

I, LUTHGARDA A. REYES, of legal age, and with residential address at Brgy.


9, Boao, Catbalogan City, Samar , under oath, hereby depose and say, that:

1. That I was issued a Philippine Passport by the Department of Foreign Affairs.

2. That said Philippine Passport is missing and must have been lost sometime
in February, 2018. Despite long and diligent efforts, it could no longer be
recovered.

3. I am executing this Affidavit to attest to the truth of the foregoing and to


support a request for replacement copy of the above-mentioned passport.
