Dear Ma'am Edna Jocelyn Caballero,

Please accept this letter as my formal resignation from being a staff nurse in Cebu Doctors'
University Hospital, effective thirty days from today, April 5, 2018.

After a period of consideration I have finally decided that I will be returning to my hometown
and have my nursing practice there. During my time at CDU-H, I have been fortunate for the
opportunity to connect and empower the lives of so many individuals and families

I am sure CDU-H will continue to live up to it's sterling reputation and continue providing top
patient care. Many thanks for the experience I had these past few months, it had prepared
me well for my next challenge.

Please let me know how can I be of help during the transition period. I wish you and the
company the very best going forward.
