AFFIDAVIT

I, ATTY. ARIEL A. ABIS, Filipino, of legal, single and with residence at Block
7 Lot 2, DJD Village, San Manuel, Puerto Princesa City, after being duly sworn
in accordance with law, depose and state:

1. That I am an owner of a Cool Silver Mitsubishi 2017 Mirage GLX 1.2G MT


with Engine No.3A92UGJ1223, Serial No. PAEA03NJHHA000118 and C/S
No. B1-J718; (Sales Invoice is herein attached as Annex “__” and Vehicle
Delivery Receipt Annex “__”)

2. That I have a Student Permit issued by the Land Transportation Office


(LTO) with Permit No. D1118000195;

3. That on or about 11:00 in the morning of March 27, 2018 my car bumped
into a concrete stand in front of the Municipal Hall of the Municipality of
Aborlan; (Pictures of the damage is herein attached as Annex “B”)

4. That at the time the incident happened it was our family friend and
designated driver DANILO D. EBUENGA with Driver’s License No.
D1182005648 who was the one driving my Cool Silver Mitsubishi 2017
Mirage GLX 1.2G MT; (O.R. of the Driver’s License of MR. DANILO D.
EBUENGA is herein attached as Annex “____” with Lic. No. D1182005648)

5. That the incident happened at the time I am on official business at the


Municipality of Aborlan;

6. That at the time incident happened I was sitting at the front passenger
seat of the Cool Silver Mitsubishi 2017 Mirage;

7. As such, I am executing this Affidavit to attest to the truth of the foregoing


and for whatever legal purposes that this may serve.
