I, __________________, of legal age, Filipino Citizen, and resident of Talipapa
Bukid, Barangay Manocmanoc, Boracay Island, Malay, Aklan, after having been sworn to in
accordance with law hereby depose and state:

That I am the owner of ______________________________ located at Stall 108


Zone 7, Manggayad, Barangay Manocmanoc, Malay, Aklan and duly registered with
Department of Trade and Industry Certificate under Registration number 04592357;

That on February 21, 2017, I operated the above-named business, which I


personally supervised and managed;

That I do not have any employee/staff/worker employed on my said business, from


the time of operation until present;

That I am an indigent, which is recorded and certified by my Barangay (Afga,


Tangalan);

That I am executing this Affidavit of Undertaking to attest the truth and veracity of the
foregoing fact and for whatever legal purpose/s it may serve.

2018 at Malay Aklan.
