and wife, respectively, and presently residing at ___________________, after
having been duly sworn to in accordance with law, depose and state:

1. That we are the biological parents of ________________ who was born


on _____________ at __________________, Palawan as evidenced by the
attached copy of the child’s Birth Certificate;

2. That as per Birth Certificate, the space pertaining to the “Father”, as


well as the portion indicating “Affidavit of Acknowledgment/Admission
of Paternity”, was left blank;

3. That to supply the necessary information and to acknowledge


paternity over the minor child __________________, a separate “Affidavit
of Acknowledgement of Paternity and Consent to Use Father’s
Surname” was executed by the undersigned ______________________;

4. That at the time of her birth, we, the biological parents were not yet
married;

5. That at the time of the conception of our child, there was no legal
impediment for us to contract marriage;

6. That we were married on ______________ at_______________, as


evidenced by the attached Marriage Certificate;

7. That by virtue of our subsequent marriage, the said child is now


legitimated by operation of law and shall be using the name
___________; and

8. That we are executing this Affidavit to attest to the foregoing facts, for
purposes of complying with the requirements in applying for the
legitimation by subsequent marriage of our daughter,
