1. Fernando Jose and Marimar jointly executed an affidavit to legitimize their son Pulgozo who was born out of wedlock. 2. At the time of Pulgozo's birth, Fernando and Marimar were not yet married but there was no legal impediment to their marriage. 3. Fernando and Marimar later married on February 14, 2015 and by virtue of their subsequent marriage, Pulgozo is now considered legitimized under the law and shall use the surname "Jose".

JOINT AFFIDAVIT OF LEGITIMATION

WE, Fernando Jose and Marimar, both of legal age, husband and wife,
respectively, and presently residing at City of Pines, after having been duly
sworn to in accordance with law, depose and state:

1. That we are the biological parents of Pulgozo who was born on June
30, 2013 at the City of Pines, evidenced by his Birth Certificate;

2. That in his Birth Certificate, no entry was registered in the portion for
the Father;

3. That to acknowledge paternity over Pulgozo a separate “Affidavit of


Acknowledgement of Paternity and Consent to Use Father’s Surname”
is being executed;

4. That at the time of his birth, we, his parents, were not yet married;

5. That at the time of the conception of our child, there was no legal
impediment for us to contract marriage;

6. That we were married on February 14, 2015 at the Pines Hotel,


evidenced our Marriage Certificate;

7. That by virtue of our subsequent marriage, Pulgozo is now legitimated


by operation of law and shall be using the name Pulgozo Jose; and

8. That we are executing this Affidavit to attest to the foregoing facts, for
purposes of complying with the requirements in applying for the
legitimation of our son, _____________________ and for all legal intents
and purposes.
