The time has come for our December field trip. The following information includes all the details
pertaining to the trip. Please read the information at the bottom of this form, then sign and return the
bottom portion of this permission slip, including trip fee(s) on/by December 10, 2019

Cost: $20 / $15 for child rental Cost: $25 for chaperones /$20 for
skates, $5 for transformation fair. adult rental skates, 5 for
transformation fair.
Place: The Rink at Rockefeller Lunch: Bag Lunch will be provided
Center 600 5th Ave, New York, NY for students, as well as snack.

*Students are expected to wear snow pants, thick socks, boots, hats, mittens and, a coat. *
Please keep the top portion of this paper at home for future reference. Cut and return the
