I understand that my child (or other minor for whom I am responsible) named below has applied
for a job with Panda Restaurant Group, Inc., or with one of its subsidiaries or affiliates
(collectively the “Company”). I understand that as part of the application process, the Company
will obtain, from a consumer reporting agency, a background check, including criminal history,
on the minor named below. I hereby consent to the Company’s obtaining a background check as
described above.
