This Deed of Extrajudicial Settlement of Estate (“Deed”), made and


entered into by and among:

DANILO CRUZ, of legal age, Filipino citizen, married, and a resident of


#15081 San Roque St., Brgy., Moonwalk, Paranaque; and

ALVIN LOPEZ CRUZ, of legal age, Filipino citizen, married, and a resident
of B2 L35 Birch St., Lancaster Lane, Alapan II-A, Imus, Cavite; and;

DARWIN LOPEZ CRUZ, of legal age, Filipino citizen, a resident of 369 Victor
St., Winnipeg, Manitoba Canada, represented by ELIZABETH LIM CRUZ,
with resident address at B2 L35 Birch St., Lancaster Lane, Alapan II-A, Imus,
Cavite;

ANALYN CRUZ ORDONEZ, of legal age, Filipino citizen, a resident of 28-


4991 Rathkeale Rd. Missisauga, Ontario, Canada, represented by
ELIZABETH LIM CRUZ, with resident address at B2 L35 Birch St.,
Lancaster Lane, Alapan II-A, Imus, Cavite;

ERWIN LOPEZ CRUZ, of legal age, Filipino citizen, a resident of 800


Cavalier Drive, Winnipeg, Manitoba Canada, represented by ELIZABETH
LIM CRUZ, with resident address at B2 L35 Birch St., Lancaster Lane,
Alapan II-A, Imus, Cavite

WITNESSETH :

THAT the above-named HEIRS are the legitimate children/heirs of


deceased ALICIA LOPEZ CRUZ, who died intestate on August 31, 2018 in
the City of Paranque. Copy of the Death Certificate is hereto attached and
marked as Annex “A”.

THAT said deceased, at the time of her death, left certain personal
properties in the form of accumulated Peso bank deposits with Banco De
Oro (BDO) located at Brgy. Moonwalk, Multinational Village, Paranaque City,
more particularly described as follows, to wit:

1. BDO – Checking Account No. 011138002352 – Amount – Forty


Eight Thousand Four Hundred Sixty One and 100/08 (P 48,461.08)

2. BDO Passbook – Amount – Thirty Thousand Eighty Two Pesos and

THAT the said deceased left a certain personal property with the San
Dionisio Credit Cooperative (SDCC) located at 0554 Quirino Avenue
Brgy. San Dionisio, Paranaque City, more particularly described as
