Our company, Semirara Mining and Power Corporation (SMPC), is engaged in the process
of vendor accreditation and updating its vendor database effective June 1, 2008 and
onwards.
On this basis, your organization is required to complete the attached Vendor Accreditation
Form and submit it to the Purchasing Department together with other specified information
and documentation:
1. The Form should be fully accomplished and signed by an authorized signatory
on behalf of the business.
2. The completed Vendor Accreditation Form and supporting documents must be
submitted in one (1) hard copy and delivered in a sealed long brown envelope
addressed to :

3. SMPC will evaluate the application and all documentation attached hereto.
Based on the results of the evaluation, the vendor shall be notified of its
accreditation or non-accreditation. The list of accredited vendors shall be
updated after evaluation of successful accreditation.
4. Failure to comply with any of the conditions as set out above and/or failure to
provide the information requested may, in SMPC’s sole discretion, result to your
non-accreditation.
5. All information submitted to SMPC and stated explicitly by the vendor in its
completed form shall be treated with confidentiality and due care by SMPC.

In cases of allegations of fraud and corruption affecting integrity of a Purchase/Sales Order,
Contract or Agreement with SMPC, SMPC and its authorized representatives shall have the
right to audit, to examine, and to make copies of or extracts from all financial and related
records [in whatever form they may be kept, whether written, electronic, or other] relating to or
pertaining to said Purchase/Sales Order, Contract or Agreement kept by or under the control of
our Company, including, but not limited to those kept by us, our employees, agents, assigns,
successors, and subcontractors. Such records shall include, but not limited to, accounting
records, all paid vouchers including those for out-of-pocket expenses; other reimbursement
supported by invoices; ledgers; cancelled checks; deposit slips; journals; original estimates;
estimating worksheets; contract amendments, change order files; timesheets; memoranda and
correspondences. Our Company shall at any time requested by SMPC, whether during or after
completion of a Purchase/Sales Order, Contract or Agreement, make such records available for
inspection and audit (including copies and extracts of records as required) by SMPC. Such
records shall be made available to SMPC during normal business hours at our office or place of
business and subject to a three day (or reasonable period) written notice. Costs of audit
conducted under the authority of this right to audit will be borne by SMPC.
