In accordance with the Revenue Regulation No. 3-2005, EO#398 and Revenue Memorandum
Order No. 10-2006, RR#10-2012, RR#10-2014, RR#1-2012 and/or RR10-2007 prescribing the
implementation of the Electronic Filing and Payment System (EFPS), I would like to inform your good
Office of my company’s intention to enroll in the Electronic Filing and Payment System (EFPS).

The purpose of enrolment is (check whichever is appropriate):


Our company is a Licensed Local contractor or is participating in public bidding
Our company is identified as a TAMP taxpayer.
Our company has a complete Computerized Accounting System.
Our company is an Accredited/Prospective Importer.
Our organization is a National Government Agency/Corporation.
Our company has a paid-up Capital of 10M and above.
Our company enjoys Fiscal Incentives.
Our company would like to volunteer to utilize the EFPS.

I also intend to enroll in the electronic payment system of EFPS-Accredited Agent Bank
Landbank of the Philippines, Camiling, Tarlac immediately or within a week after ‘On-line
enrolment’, and Activation by the BIR-TSS Chief. I am submitting herewith copies of DTI Certificate;
Updated/Latest Certificate of Registration (COR-BIR Form 2303); and/or BIR Form 1905 (Taxpayer
Registration Update Form) to include Tax Form/s_______ in my tax return compliance. I am also
authorizing Marycris B. Caoile to undertake the EFPS transaction.
