I, PRINCESS CINDERELLA, Filipino, of legal age, married, with 3


children, a resident of 456 Alice Street, Barangay Wonderland,
Manila, Philippines, after being duly sworn in accordance with
law, and by way of answers (A) to questions (Q), depose and
say:

Q1: Do you swear to tell the truth and nothing but the truth
in the taking of this judicial affidavit?

A1: Yes Sir.

Q2: For the record, please state your personal


circumstances.
A2: I am PRINCESS CINDERELLA, Filipino, of legal age,
married, with 3 children, a resident of 456 Alice Street,
Barangay Wonderland, Manila, Philippines

Q3: Why are you here at the law office?

A3: To give my statement regarding the facts I personally


know with respect to this case filed against PRINCE
PHILIP SALVADOR by private complainant PRINCESS
AURORA AVENUE and to execute my judicial affidavit.

Q4: Do you know Prince Philip Salvador?

A4: Yes

Q5: How did you know him?

A5: He’s the grab driver we booked last July 4, 2019.

Q6: Do you remember what time was your transaction with


him?

A6: It’s around 12 midnight when I booked him and was


dropped off at 12:50.

Q7: Were you alone that time?

A7: No. I was with my son.

Q8: How was your trip then?

A8: It was okay with me. But my son felt dizzy and nauseous,
because he does not like smell of cars. He eventually Commented [S1]: Please rephrase. Anong mas maganda?

vomited.

Q9: Do you remember where specifically the vomit was?


A9: It was all over the back seat because he was lying facing
the “sandalan” of the car. Commented [S2]: Or mas okay ba rear?

Q10: What did you do after?

A10: I said sorry to the Prince Philip and tried to clean up the
mess.

Q11: What did Prince Philip said?

A11: That it was okay and that he understand. He also


suggested that we’ll just clean it up upon dropping us off
so the car is at full stop.

Q12: Do you swear that you answered the questions asked of


you now, fully conscious that you are under oath, and
that you may face criminal liability for false testimony or
perjury?

A12: Yes, I do.

Q13: Do you have anything more to add, delete or remove in


this Judicial Affidavit?

A13: No more, Sir.
