MRCPCH Clinical Exam Circuit > = MRCPCH = Clinical Circuit xRCPCH Changes to the MRCPCH Clinical Circuit Timings: * Circuit still has 9 and 22 minutes stations. * The 9-minute stations are now split into 6 + 3 minutes. + The 22 minutes stations are still 13 + 9 minutes. * Warning knocks are given 3 minutes before the end of a station. 3 station changes: * Clinical stations + Video station + Development station * No significant modifications made to Communication and History stations *RCPCH MRCPCH Clinical Exam Circuit + 12 candidates in 1 circuit - 24 per day * 10 exam stations + 2 Development stations & 2 History stations run at same time (Red & Blue) * 4 Clinical stations * 2 Communication stations * 2 Video stations * 4minutes between stations *RCPCH MRCPCH Clinical Domains 5 principal domains. Domains A, D and E have sub-domains. | Domain | Descriptor/Domain A Information gathering/ sharing B Physical Examination c Identification of Clinical Signs D Clinical Reasoning and Management Planning - Pr E Communication *RCPCH MRCPCH Clinical: Pass Mark Information + Across 10 stations, there are 41 domains. * Each domain has a maximum mark of 2. + Maximum total marks available = 82. + Pass mark set at 63 marks. + Determined using criterion referenced method called Modified Angoff. *«RCPCH Domain Mark: Rating scale explained - Meets Standard The ‘Meets Standard’ mark is not a mark of excellence. Should be awarded when candidate achieves key criteria listed on anchor statements/mark sheets plus additional criteria included in either: 1. The examiners pre-determined central benchmarking guidance (History, Communication, Video stations) 2. The agreed benchmarking criteria recorded by examiners (Clinical 1-4, Development stations) Examiners may not provide feedback on a domain on back of mark sheet when awarding ‘Meets Standard’ mark. Refer to anchor statement / mark sheet domain criteria listed for ‘Meets Standard’. *RCPCH Domain Mark: Rating scale explained - Below Standard A ‘Below Standard’ mark awarded indicates that a candidate's performance has not met the required standard for the domain; this mark is equivalent to a fail for the domain. Where ‘Below Standard’ mark is provided, the examiner is trained to tick the relevant feedback box on back of mark sheet to support judgement. It is hoped that this will provide useful feedback to candidates. If there is enough time between stations, examiners will attempt to include free text comments on back of sheet to provide further feedback on domain marks *RCPCH After the exam RCPCH Quality Assurance - Process to review results, complaints and reports. To ensure exam is reliable, fair and valid. + CEB - Clinical Exam Board Meeting (approx. 5 weeks after each exam) + Appeals/Complaints Panel (approx. 7-8 weeks after each exam). Focus of Appeals Panel is on procedural irregularities and unexpected incidents during exam days. Exam results cannot be altered. *«RCPCH cares 1g o0pIA, vy o9pin g sumo ysunu0g AsosiH quauidoyanag leo, e1eKND. z reat b penn St2 St3 St4 StS Sté St7 sts sto st1o stt RCPCH Standard of the exam - RCPCH Progress + MRCPCH Clinical Exam mapped to Progress curriculum. + Assesses 3 of 1! learning outcomes in Level 1 Generic Syllabus “standard of clinical skills expected of a trainee entering Level 2 training - sta* For more information on the Progress Curriculum ‘Level 1 Generic Syllabus’ and the standards expected to complete Level 2 training please refer to the RCPCH website: RCPCH Progress curriculum and generic syllabi repch_progress_curriculum_level_1_generic_syllabus_for_use_from_1_aug_2018. pdt *RCPCH Progress Paediatr curcuhm fr excelence *RCPCH MRCPCH Clinical Domains explained The way we assess candidates in the exam has changed; we now assess according to clinical domains. MRCPCH Clinical Domains outline the core skills we expect candidates to demonstrate across all stations. Examiners make focused judgments about a candidate's performance across these ‘domains’. Relevant Domains are tested based on specified tasks in each station. Domain Based approach: + brings more clarity to what is being assessed across all station types. + focuses examiners on domain competency - not a global judgement of performance. + improves standardisation across examiners and exam centres making the exam fairer. + provides focused feedback to candidates. *«RCPCH Development Station Children used in this station should have a developmental age of less than 5 years. * Aim Q To examine the candidates ability to assess key areas of development, and take a focused history to explore reasons for these findings, including the impact on the child and family. + Timing U1 22 minutes for station O 13 minutes with patient and parent/carer Q 9 minutes with examiner for discussion Q 3 minute warning from end of station *RCPCH Clinical Stations (1-4) + Stations no longer focused on pre-specified set of systems (CVS/Resp..etc.) + Candidates can potentially be presented with any kind of system focus/case. + Selection / focus of cases in each of Clinical 1-4 dependant on the host centres patient availability/recruitment. + Examiner to provide clear introduction to task/case to each candidate including what the system focus is e.g. Respiratory system. + Important - Candidates are not expected to provide a management plan in clinical station *RCPCH Loe PLEA | Screen aye Specific domains tested in Development station #RCPCH All mark sheets will be returned to the RCPCH after each exam day * Copies of mark sheets will be provided to candidates once results are published * Candidates should reflect on feedback received *RCPCH The mark sheet + Check that you have given the correct mark sheet for your station to the examiner when you enter the room. + All mark sheets will be returned to the RCPCH after each exam day + Copies of mark sheets will be provided to candidates once results are published + Candidates should reflect on feedback received *RCPCH rr Candidate Feedback Domain marking helps to crystallise thoughts for written feedback - aims to help candidates to be aware of strengths and weaknesses including which areas to focus on for improvement. Examiners are trained to provide feedback on the back of each mark sheet if the mark is less than ‘Meets Standard’, by ticking the relevant box. Additional feedback in free text boxes can help a candidate to better prepare for a future exam if they have to resit. Mark sheets include the anchor statement scoring criteria for each domain in order to make it easier to provide this feedback. Available on the back of all mark sheets. *«RCPCH aaa Unprofessional Conduct Unprofessional Conduct concern will not count towards a candidate's final mark - Any recorded unprofessional conduct marks will be reviewed/discussed during the senior examiner de-brief and again at the relevant Clinical Exam Board meeting (CEB) after the exam day. If an examiner determines that a candidate is rough with a patient or that a patient appears uncomfortable or their safety is at risk it is likely that this will be noted as unprofessional conduct. Other examples of unprofessional conduct could include rudeness, inappropriate language or dangerous advise or unsafe proposals of treatment / management etc. *RCPCH Marking: Mark Sheet Example Section (Front) If a candidate demonstrates unprofessional behaviour examiners will tick YES here and provide detail of their concerns. They will also discuss with the Senior Examiner. *RCPCH Candidate Conduct: Interaction with others + Everyone is there helping to facilitate the exam. * Be polite and respect all individuals you come into contact with. + Be professional and friendly. *RCPCH On the day of the exam + Leave plenty of time to get to the exam centre - arrive no later than 09:00. + Upon arrival register for your exam. + Switch off your mobile. + Raise any questions or concerns with Senior only at the end of your exam + Ifyou have a concern or complaint contact the Exams Team within 72 hours of your exam *«RCPCH rr On the day of the exam * Leave plenty of time to get to the exam centre - arrive no later than 09:00. + Upon arrival register for your exam. + Switch off your mobile. + Raise any questions or concerns with Senior only at the end of your exam + Ifyou have a concern or complaint contact the Exams Team within 72 hours of your exam *RCPCH rr
