Chief complaint : He kept on repeating the words

PERSONAL HISTORY :
The patient complained that it was his mother who brought
him to the hospital.He was quiet as we ask questions to him.No sexual
relationship yet.after somtime he said that he has four brothers and all of them
come and visits him.

FAMILY HISTORY :
He said that he kept on asking his mother for money and she
was not okay with that, but his father was fine with him.he could remember all
his brothers name.

MEDICAL HISTORY :
The patient already attends counselling from the
doctors.He mentioned that he is currently taking medications , but not mention
their names.

APPEARANCE : well dressed, clean, nails perfect


BEHAVIOUR : normal and no risk of violent behaviour
