SUBJECT: PROPOSED GEO-TECHNICAL INVESTIGATION OF FIVE (5) BOREHOLES AT 15.00 METERS
DEPTH LOCATED AT BASAK, LAPU-LAPU CITY, CEBU
Dear Sir,
With reference to the above subject, we are pleased to submit our geotechnical investigation proposal for your
evaluation and consideration.
A. SCOPE OF WORK:
    1. Drilling of five (5) boreholes of 15.00 meters depth. The boreholes shall be drilled at the Client’s
       Specified/marked locations. However, this assumption may change depending on the actual site
       condition such as relative areas, height and distance between areas that are to be drilled upon final
       confirmation from Client’s representative.
    2. Conduct Standard Penetration Test (SPT) including disturbed sampling every 1.50 meters interval
       including water table monitoring/recording for each on-shore borehole only.
    3. Retrieve undisturbed samples on cohesive soils encounter using standard thin walled tube (Shelby)
       sampler.
    4. Should hard strata or rock formation is encountered, coring will be conducted and samples will be
       taken. The drilling process will be terminated then at maximum of 5.00 meters coring depth. Rock
       Quality Designation (RQD) shall be determined and other necessary physical properties.
    5. Conduct laboratory testing on selected soil and rock samples taken from the boring/coring as follows:
                 a. Natural Moisture Content (ASTM D-421)
                 b. Sieve analysis (ASTM D-422)
                 c. Atterberg Limits (ASTM D-4318)
                 d. Unified Soil Classification System (ASTM D-2487)
                 e. Unconfined Compression Test (ASTM D-2166)
    6. Preparation and submission of three (3) copies of geotechnical report signed/sealed by our
       geotechnical engineer/evaluator which will contain the following
       a. Soil profile, type and classification, SPT N-values and water table location for on-shore test holes if
          encountered. Stratification of the area explored is limited through the drilling depth explored.
       b. Discussion on soil stratification and other technical information relevant to the design of the
          structural foundation within the area.
       c. Evaluation and recommendation of the soil bearing capacity for the different footing sizes and
          depths for shallow and deep foundation.
       d. Evaluation of site liquefaction potential should the site be susceptible to this phenomenon.
       e. Photos of soil exploration and soil samples per test hole.

The Lump Sum Services is Three Hundred Eighty Six Thousand Two Hundred Thirty Two Pesos

C. TERMS AND CONDITIONS:
  1. Work shall commerce upon receipt of Notice-to-Proceed (NTP) or Job Order (JO) with
     corresponding fifty (50%) percent down payment of the total project cost.
  2. The client shall provide site development plan with location of boreholes and shall locate and mark
     test/boreholes jointly with MTCI, Secure permit if necessary, Facilitate gate passes needed by our filed
     crews to access the site.
  3. The client shall allow our crew to stay at the site and provide or security to ensure safety of our,
     equipment, tools and personnel/staff, a temporary area/facility with water, power, a comfort room
     “free of charge” during the entire duration of drilling or field works as these were not charge either.
  4. Test holes shall be passable by 6 wheelers elf & free of any obstruction and concrete cover prior to our
     mobilization. MTCI shall not in any way be held liable for any accident or damage in the drilling of the
     located/marked boreholes.
  5. Availability: subject for schedule
  6. Price Validity: good for two (2) months upon received of this price quotation
  7. Mode of Payments:
     a. Down/Advance payment: Fifty Percent (50%) of the total price before mobilization.
     b. Full Payment: Fifty Percent (50%) before claiming of Final Geotechnical Report at MTCI office.
  8. Completion time: twenty five - forty (25-40) good working days from date of mobilization (field,
     laboratory & Office evaluation works)
