that also stores water when it rains.
 Bathroom is always soaking wet.
 Containers at the back sotre dirty water used
for flushing.
 Leaving dishes in sink for a long period of time.
Subjective:

 Client verbalized “ ga dagan jud sa amo Improper Diet as health

 Client verbalized “ na admit ko atong ni agi

tungod kay ge UTI ko.”

 “ wala na mi time mag luto mag sig ra mig palit

ug pagkaon sa gawas or kanang mga dali luton

nga sud.an example kay bulad,noodle,pancit

canton ug prito prito” as stated by the

daughter.

Objective:

 Excessive sweating

 Bottles of alcohol are seen at the back along

with different soft drink bottles.

 Excessive sweating.

 Repetitive use of frying oil as seen that

darkened oil is stored beside the stove.


Subjective:

 Daughter in Law verbalized “ ang isa ka anak Pregnancy/forthcoming

sa igsuon sa akong bana kay namatay 9 delivery as a source of

months after sya ge panganak kay mubo ug anxiety or stress

potassium unya sigeg sakit.” points / foreseeable

 Daughter continued to verbalize, “Habang ge crisis

buntis man gud to sya na bata grabe ka baba

ang potassium pud sa ako igsuon. “

 Client verbalized “ daghan na kaayog bata mag

libog na mi ug kinsa mag bantay.”

 Daughter also verbalized “sukad na namatay

tong anak sa ako igsuon ma hadlok mai

manganak kay basin mamatay rapud”

 Client also verbalized “ naa mi igsuon na patay

jud sya pag anak ni mama”

Objective:

 Children running around the house carelessly.

 Anxiety was present in the facial expression of

the client and his family when they were talking

about the said topic.

 Concerned about the welfare of children upon

labor and delivery if one of them becomes


pregnant

 Worries on who would look after the children

and keep them safe.

 Children lacks discipline
