A. COMPANY PROFILE
EMILIO AGUINALDO COLLEGE MEDICAL CENTER-CAVITE (EACMC-C)
Emilio Aguinaldo College Medical Center-Cavite (EACMC-C), a 154-bed private Level
2 and PhilHealth-Accredited hospital which provides in-patient and out-patient services, clinical
consultations, diagnostic services, industrial medicine, medical social service, dietary services,
pharmaceutical services, and many more.
Their services are available at a standardized and worthwhile rate for every patient’s
convenience. They also accept over 10 different health cards and has a Phil-Health Department
to lend ease in filing and processing the documents needed by our patients.
Considering the location of the hospital, most of the time emergency cases are instantly
acted upon by our Emergency staffs to provide fast diagnosis, treatment and relief to the patient

MISSION
EAC Medical Center-Cavite aims to make quality service and health care accessible to all

D. HISTORY OF THE COMPANY
Emilio Aguinaldo College Medical Center-Cavite (EACMC-C) was conceived and
established by Dr. Paulo C. Campos, driven by his ardent love for and commitment to welfare of
his province-mates. It all started from a five-year development program in 2002 for the health
allied program of Emilio Aguinaldo College (EAC).On July 27, 2006, the hospital was
inaugurated as Emilio Aguinaldo College Medical Center-Cavite and its ground were opened
offering services like OPD Maternity Clinic, X-ray, laboratory and physical therapy services
which was attended by Sec. Duque, former President Ramos and local Officials of Dasmariñas
Cavite. It was Dr. Emmanuel Campaña who was put into office as the first Medical Director.On
October 5, 2006, the hospital launched the first medical mission for the benefit of the indigent
patients of Dasmariñas Cavite which showcased free physical examinations, dental services,
bone scanning, medical, surgical, pediatrics and ob-gyne check–ups.
Later on June 1, 2007, the hospital was able to acquire a DOH license for OPD maternity
Hospital offering additional services like pre-natal services, immunization, medical check-ups,
well-baby check–up, annual physical examination for employees and laboratory examinations.
On August 14, 2008, DOH released EACMC-C’s license to operate as a tertiary hospital with
154 bed capacity for adult and children patients granting full autonomy to practice medical
procedures like major operations, etc.
EACMC-C is currently a 154-bed private tertiary Philhealth accredited hospital with a
triple A avocation to provide Advanced Accessible and Affordable medical & paramedical care
to the community. It is ideally positioned to answer the industrial needs of employees of multi-
nationals, manufacturing and processing centers that have located themselves in the

CALABARZON area. Health packages for companies, offices and schools are also available and
created to support the current thrust of preventive care and health maintenance. All these services
are delivered by highly trained and compassionate professionals.
Their close affiliation with Medical Center Manila (MCM) ensures the same high quality
care that our institutional family has long been known for.
EACMC-C’s commitment to giving the best quality care possible regardless of one’s
status in life has not changed to this day, inspired by its founder, Dr. Paulo Campos, remains to
prioritize patients first above all. Its present set of officers, administration and staffs strongly
foresee EACMCC as a well-established hospital that will rise to be one of the best hospitals

COMPANY RULES AND REGULATIONS
ADMISSION
At EAC Medical Center-Cavite, you can choose from the following rooms: Suite,
Private, Semi-private and Ward. The Admitting Office is open 24 hours a day; room reservation
may be done 2 to 4 hours prior to admission.
Health Maintenance Organization (HMO) patients are required to bring the Admitting
Orders of their Attending Physician and the Letter of Authorization (LOA) in compliance with
the “EACMC-C Admitting Policy”. However, during emergency cases the aforesaid
requirements could be nullified for the first 24 hours. Any relative of an HMO patient should be
able to complete the requirements and submit it at the Admitting Office immediately.
DEPOSITS
You are required to make an initial deposit upon admission (except in cases of emergency
or as prescribed by law. On the other hand, if you are an HMO patient (or your Company has
MOA with the hospital) your deposit will be subjected to the Hospital Schedule of Deposit.
Arrival at the nursing unit
Please present your Admitting Orders and Record to the Nursing Staff at the floor where
you are admitted before going to your room. A Nursing Assistant will orient you the different
facilities in your room and a patient’s kit will also be provided to you.

PROCEDURES TO BE FOLLOWED ON DISCHARGE
Your Attending Physician must give the WRITTEN ORDER in the chart for your
discharge.
The nurse will present the accomplished DISCHARGE AND CLEARANCE SLIP to the
billing section who in turn will prepare your final hospital obligations including the
hospital bill and the professional fee/s.
The billing section will be the one to specify the TIME SCHEDULE FOR PAYMENT.
The nurse will give you the filled–up discharge and clearance slip.
Present the discharge and clearance slip to the billing section ON THE TIME
SPECIFIED. (DO NOT LOSE THE SLIP)
The Billing Section will explain the composition of your final bills and forward it to the
cashier for payment.
Cashier will issue an official receipt once payment has been made. The cashier signs at
the discharge and clearance slip and returns the discharge and clearance slip to you.
Present the signed discharge and clearance slip to the nurse station which in turn will be
filled up by the nurse on duty before leaving the room / ward. (Nurse will get the chart
copy)
Present the discharge and clearance slip at the Security personnel.
Security personnel will check your baggage and will permit your discharge after
inspection.
Security personnel will sign at the discharge and clearance slip and will forward it to the
Admitting Section to encode discharge date and time and for filing.
