your final year may not have been encoded yet at the time of application. This is
acceptable. However, please note that if you should get a failing grade on your
last year, you may forfeit your slot.) (NOTE: Applicants from UST who have not
taken any subject/s outside UST need NOT submit a copy of their transcripts of
records.)
4 Photocopy of parent’s/parents’ and/or sibling’s/siblings’ diploma/s if UST graduate/s.
5 Photocopy of NMAT scores. Please check the list of NMAT dates that we will
considered for the present application process.
6 Two (2) certificates of good moral character (cf. Admission Policies #6)
7 Accomplished printed copy of the Online Application Form
8 Accomplished printed copy of the Health Questionnaire Form duly signed by a
licensed Physician (2 pages printed back-to-back)
9 Photocopy of Baptismal certificate (if applicable)
10 Photocopy of certificates of extracurricular activities (leadership/civic/volunteer

C. Additional Requirements
1. Transfer credential certificate from the last school attended
2. Required NMAT percentile score (valid within the last 2 years)
3. Passed all qualifying/validating examinations and interview
D. Fill up Application Form online, print out, sign and attach listed requirements.
E. Submit accomplished application form and requirements on/or before the set deadline to the Admissions
Office.
F. Secure payment slip and pay non-refundable and non-transferable Application Fee at the Cashier (2nd Flr.
Admin Bldg.).
G. Take the UERM Admission Test (if applicable).
H. Secure schedule for interview from the Admissions Office. Schedule will also be posted
at https://uerm.edu.ph/ .
I. Check status of admission at Admission Portal . You might also receive SMS or email notice.
J. Pay the non-refundable and non-transferable reservation fee on the scheduled period. The amount will
depend on the accepting college.
K. Enroll on the scheduled period of enrolment.
