THIS FURTHER CERTIFIES THAT the aforementioned is an Overseas Filipino Worker


(OFW) and a resident in this Barangay and were affected by the series of Earthquakes last
October 2019.

This certification is being issued upon the request of the aforementioned in


connection with the compliance of the requirements for OWWA Calamity Assistance
