May I have the honor to request from your good office for a transfer of teaching station
from Mariano Baron Elementary School of Magalang North District to San Miguel
Elementary School of Magalang North District.

1) That I am a bona fide resident of Barangay San Miguel, Magalang, Pampanga

2) Since that I am living at Nicolas 2 of Magalang, Pampanga. Transferring to Sta.


Lucia Elementary School will be a great help to me because it will lessen my time
in traveling from San Nicolas 2 to San Ildelfonso, Sitio Cabayu Sarul Maragul
Magalang Pampanga which is 15 kilometers away and it takes 40 minutes from
which I am a residence while San Nicolas 2 to Sta. Lucia Elementary School is
only 4 kilometers.

3) That I am living with my Grandparents and their health condition is need my


constant attention and I am the only responsible person in my family to take care
of them.

It is my fervent hope and prayer that this humble request will be


given meritorious favor from your very good office.
