POLICE REPORT

Nature of the Case : Malicious Mischief (Hit and Run)

Complainant : MARIVIC GUZMAN y ESPEJO, 33 years old, single, Billing Analyst,


native of Tarlac, and presently residing at No. 9 Rambutan
St., North Signal Village, Taguig City.

Suspect/s : One male person driving a tricycle with side car (Unidentified)

Date/Time and Place


Of Occurrence : December 1, 2018, on or about 8:00 pm., parked unattended along
Santol Ext., Sta. Mesa, Manila.

DAMAGED : Dented, scratches left fender and detached, broken left side mirror of
Toyota Vios, color metallic gray, model 2016, with conduction
sticker No. YU6399.

Brief Facts of the Case:

Herein complainant personally came to this station and reported the above captioned case for
investigation.

It appears from the narration of the reportee, that on the given date, time, place of occurrence
driver one (1) Bryan Guzman parked the vehicle unattended a Toyota Vios, model 2016, color
metallic. Gray, with conduction sticker No. YU6399 along Santol Ext. Sta. Mesa, Manila to have a
dinner together with my family at the nearby Jollibee Food store, after an hour, when they returned
and about to use the vehicle she noticed a dented and scratches at the left side fender and detached,
broken left side mirror of the vehicle.

Inquiries made by herein complainant to identify the perpetrator/s according to a certain alyas
Boy, newspaper man, it was a passenger tricycle, color orange without plate number who collide with,
and incur damages.

This Police Report is being issued upon the request of Miss Guzman for whatever legal
purposes it may serve within the premise.
