-If vessel have Indonesia Health Book/Green Book on board, if you don’t
have it so please informs us in order we can instruct local agent to
arrange it when vessel arrival because its compulsory for in/out
clearance arrangement.
-If vessel have derrating and medicine certificate on board, if you
don’t have it so please informs us in order we can instruct local agent
to arrange it when vessel arrival because its compulsory for in/out
clearance arrangement.
Note:
-Please ensure no supernumerary on board to avoid any problem with
immigration.
-Please ensure total crew on immigration crew list when departure is
same amount on crew list when arrival
-Please make sure vessel have valid certificate on board and for expiry
date of certificate should not less than six (6) months from date of
vessel arrival
-All crew passports expiry date should not less than six (6) months
from date of vessel arrival
-For PSC certificate the last inspection date should not less than six
(6) months from date of vessel arrival
-The PSC certificate must be issued by member of Tokyo MOU
-Last port clearance must be declared Panjang / Lampung as next port
