A student was injured while swimming in class. A small sharp object called a steel pin punctured the student. The swimming pool in-charges vacuum the pool regularly but small objects like pins can get stuck between tiles. They will do their best to prevent future incidents and have requested approval to replace the pool water which has been used for 11 months.

Dear Sir,

We are writing to report/inform your good office about the incident happened last Wednesday
morning, September 11, 2019. Early in the morning yesterday, the class of Mrs. Daluro had their
P.E. While the class is going on, an unexpected incident happened. One of the students of Mrs.
Daluro was punctured by a small sharp object called “steel pin” while swimming. He was
brought to the clinic and eventually to the nearest hospital for a medical treatment.

As the swimming pool in-charge, we are saddened about this first ever minor accident
happened in our premises. As far as our roles and responsibilities are concerned, we would like
to inform you that we vacuum the pool every other morning to assure that the pool is clean and
safe to use. Unfortunately, a small object like a pin is somehow hard to vacuum because it
sticks between the tiles. There is a high probability of having pins in the pool since the
swimming pool area is being used as venue for some school events and even events of other
institutions/organizations. Organizers always use pins for skirting and/or for decorating. In our
own observation, this might be the cause of the said incident.

Nevertheless, we would like to reassure you that we will do our best in work to avoid any
untoward incident again.

I hope that this report/letter fully explained the said incident.

We, the swimming pool in-charge, would like to request for your approval in water replacement
of the swimming pool. As per water treatment and sanitation are concerned, the said water
needs to be replaced because it is being used for 11 months already.

Moreover, to make this realize, we would like to ask also for permission to close the swimming
pool starting September 13, 2019- September 22, 2019.
