RELEASE AND RETRIEVAL OF MODULES PROCESS FLOW

I. Release

1. Pre-activity

a. Adviser prepared the module bundle

b. Teacher arrives 15 minutes prior to scheduled time

2. Upon entering the school premises

a. No ID, No Facemask, No Entry

b. Temperature will be checked and recorded

c. Log in to log book (designated table per strand)

d. Physical distancing should be observed

e. Hand Sanitation

3. If CLEARED

a. Proceed to classroom

b. Only 20 students are allowed to enter the room

c. Physical Distancing should be observed

d. Hand Sanitation

e. Orientation via school radio broadcasting is to be administered

f. Modular Instruction/Discussion to students through adviser in-charge is to be

conducted

g. Release of module bundle 1

h. Student shall leave the school premises once modules were received

4. If NOT CLEARED

a. If symptom is apparent upon conducting the health and safety protocal before

entering premises, the student shall be referred directly to the school clinic for

further assessment.

b. If patients do not need any further asssistance based on clinic’s assessment, they

may proceed to the classroom and follow minimum health requirements.

c. If patients show symptoms, they will be advised to go home and seek advice to

the medical authorities.

I. Retrieve

1. Upon entering the school premises

a. No ID, No Facemask, No Entry

b. Temperature will be checked and recorded

c. Log in to log book (designated table per strand)

d. Physical distancing should be observed

e. Hand Sanitation

2. If CLEARED

a. Proceed to classroom

b. Only 20 students are allowed to enter the room

c. Physical Distancing should be observed

e. Adviser should collect the module and give to Subject Teachers for checking (A

week after the released/or as the subject required based on it’s duration in the

K-12 MELCs)

f. Students shall leave the school premises once module were rendered

3. If NOT CLEARED

a. If symptom is apparent upon conducting the health and safety protocal before

entering premises, the student shall be referred directly to the school clinic for

further assessment.

may proceed to the classroom and follow minimum health requirements.

Note: In case the student failed to secure or return the modules at the school premises, the

adviser should coordinate to the barangay sector on how to release and retrieve the

modules as an alternative actions. (Each barangay is requested to send 1 kagawad-the

education sector represenatative to the school premises to do the tasks)
