Rubric For Nursing Care Plan

This rubric outlines grading criteria for a nursing care plan with five categories: assessment, diagnosis, planning, implementation, and evaluation. For each category, it describes the elements needed for an exemplary (5 points), proficient (4 points), or needs improvement (3 points) score.
