I. HISTORY

This office is one among the offices devolved to the Local Government Unit of tuba
pursuant to the Local Government Code of 1991. Prior to the devolution, MSWD office existed
already but under the National agency called the Department of Social Welfare and
Development.

The office is the primary welfare office of the Municipality committed to the care,
protection, and rehabilitation of the marginalized sector of the populace that need welfare
assistance and social work interventions to restore their normal social functioning and
participation in community development. It capitalizes on human strengths through organization
of individuals and delivery of services through a community-based approach with the endearing
support of the Local Government Officials towards developmental growth of the marginalized
groups.

II. VISION, MISSION GOALS

VISION

We envision Atok to be a progressive community of empowered healthy and God loving citizens
doing viable, profitable and sustainable countryside development programs in an ecologically
balanced and peaceful environment

MISSION

To come up with a comprehensive citizens’ charter that will serve as a guide for policy making
and for delivery of basic services

GOAL

Improved quality of life through equitable access to basic services and facilities

injuries & casualties, families in evacuation centers)

within the family; couples applying for a marriage license

VI. POLICIES & PROCEDURES
1. In providing Medical Assistance & Burial Assistance, the service provider should receive &
evaluate submitted requirements by conducting an intake/ interview of client; setting a
schedule of home visit; and evaluating if client is eligible for assistance. After that, the
service provider will conduct a home visit and prepare a case study report and supporting
documents (forward to LGU offices for review, approval, and release of funds). Lastly,
inform the client as to the availability & release of funds.
Documents Needed: Death certificate; Certification of Membership from the Veterans
President; Barangay Certification as to residency; & Intake form
2. In the provision of Transportation assistance for stranded victims and for abuse cases filed at
the Fiscals/ Court Office, the service provider will conduct an intake/interview of the client
and he/she will prepare a case study report and supporting documents (forward to LGU
offices for review, approval, and release of funds).
Documents Needed: Police Investigation Report for abuse cases and stranded victims &
intake form
3. For educational assistance: Receive & evaluate submitted requirements (Conduct
intake/interview of client; set schedule of home visit- if client is found eligible). Conduct
home visit. Prepare case study report and supporting documents (forward to LGU offices for
review, approval, and release of funds). Inform client as to the availability & release of
funds.
Documents Needed: Barangay Certification (as to residency and indigence); School
Certificate where applicant is currently enrolled in & Intake form
4. For Pre-marriage Counseling: Assist client to answer the Marriage Expectation Inventory;
Conduct of Pre-marriage session; Type Pre-marriage Counseling certificate for issuance
Documents Needed: Official receipt from the Treasury Office
5. For Food for Work assistance for disaster related activities: Receive & evaluate submitted
request endorsed by the mayor; release of goods
Documents Needed: Approved letter of request
6. Disaster relief and Assistance to Calamity Victims: Conducts interview/intake of client;
Release of goods; & assessment of damages aftermath of disaster for rehabilitation services
7. For Crisis intervention victim for victims of abuse & exploitation: VAWC (Assist client for
Medical and Legal Assistance & Provide Counseling to client by referring her/him for
temporary custody/shelter, then prepare Case Study report)
Documents Needed: Sworn stamen of the client taken by the Police Women’s Desk Officer
& Medical Certificate
CICL: Intake & interview of client; Conduct home visit; prepare Case Study report; Assist in
the preparation of an Intervention plan for 15 yrs. Old below and Diversion program for 15
yrs old above but below 18 yrs. Old.
Documents Needed: Report from the Barangay Council for the protection of Children on the
incident that happened & Police Investigation report on the incident
8. Supplemental Feeding for malnourished children: Receive & evaluate submitted documents;
Prepare documents and forward to concern LGU offices for review, approval & release of
funds; & Purchase and release of supplemental goods.
Documents Needed: Letter of request and master list of malnourished Children.
9. Financial Assistance for Livelihood Projects: Receive & evaluate submitted documents
(Conduct interview/intake of client); Conduct Home visit; Prepare documents and forward to
concern LGU offices for review, approval and release of funds; Inform the client as to the
availability & release of funds
Documents Needed: Project Proposal; Financial Statement
